The single fee amount includes: clinic staff administrative services; referral of the patient to the examining and treating physician; medical laboratory blood testing at the beginning and prior to the end of therapy; physician services provided in conducting your physical examination; physician services rendered in performing the clinical assessment and evaluation of the patient; physician services performed in prescribing indicated treatment; preparation and deliver of physician's written treatment instructions; independent pharmacy services; and pharmacy dispensed medication shipped and delivered directly to the patient by overnight delivery. There are no additional costs unless the patient requests additional services of medication ordered or prescribed as indicated by the patient's NMC treating physician.

If sought treatment is declined by the evaluating physician, the patient's purchase funds are refunded to the patient less the fees for physician referral, clinic services, laboratory blood testing, and the fees relating to the physical examination and evaluation of the patient by the physician to whom the patient is referred.

A Valid Prescriptions Based Upon the Physician's Physical Examination and Evaluation of the Patient are Required for all National Medical Clinic, Inc.'s Physician Prescribed Treatments

National Medical Clinic requires that its physicians obtain a medical complaint and medical history from each of patient, conduct a physical examination, perform a patient clinical assessment, complete a patient evaluation, render a diagnosis and obtain an indication that that the medication is prescribed for a valid medical purpose in the normal course of professional practice prior to prescribing treatment. NMC physicians do not prescribe any controlled substance prescription drugs to patients referred to them by National Medical Clinic, Inc. NMC physicians utilize medical treatments that cause the human body to naturally produce the desired treatment result where possible; i.e., use of Sermorelin Acetate growth hormone - releasing hormone to cause the pituitary gland to increase the natural production of human growth hormone within the protections afforded the patient by their endocrine system; and the administration of human chorionic gonadotropin (HCG) to stimulate the male testes to increase natural production of testosterone and to treat male infertility. Prescription drugs with significant adverse side effects are not to be prescribed patients of National Medical Clinic, Inc.
